To be eligible, patients must meet the medical criteria for testing (see link below), and meet low income requirements (household incomes up to 400 percent of the Federal poverty level). Due to regulatory limitations, patients who are recipients of government funded programs (e.g., Medicaid, Medicare, Medicare-Advantage, and Tricare) are not eligible to apply for MFAP.
