Several years ago, a patient came into the office for an entirely different concern – she had experienced a miscarriage. As Linda reviewed her chart, something in the patient’s family history stood out: both her paternal grandmother and paternal aunt had breast cancer. Linda hesitated for a moment. Was this really the right time to bring up hereditary cancer risk? The patient was already going through so much. Still, Linda trusted her instincts. When she mentioned her concerns, the patient explained that she was already seeing a breast specialist. Linda felt reassured — until the patient shared that the specialist told her she didn’t need to worry about her paternal health history, only her maternal side. Linda took a moment to clarify that inherited genetic risk can come from either side of the family, and suggested that since they were already drawing blood for miscarriage-related testing, they could also run a hereditary cancer test. The results came back positive for a BRCA2 gene mutation. With that knowledge, the patient decided to pursue increased surveillance — a crucial step as she and her partner continued trying to grow their family. “Would you normally talk about cancer and hereditary testing with someone experiencing a miscarriage?” Linda reflected. “Maybe not. It can feel uncomfortable. But I did — and she tested positive. Every patient. Every visit.”
