4mL blood collection kit instructions

Before you start

This kit is to be used for the following tests:

• MyRisk^® Hereditary Cancer Test 
• Foresight^® Carrier Screen

If the patient has had a transfusion within the past seven days, wait to collect the patient’s sample until seven days have passed. If the sample cannot be shipped on the same day as it is drawn, please refrigerate the sample until it can be shipped.

Step 1

Write the patient’s full name, date of birth, collection date, and phlebotomist initials on EDTA tube

Step 2

Draw blood from patient. Fill collection tube with at least a 4mL sample. After sample collection, gently invert tube several times.

Step 3

Seal the tube in the specimen bag. Place the specimen bag in the kit box. Give the patient card with the unique sample barcode to the patient for his or her records.

Step 4

Seal the kit box in the FedEx Clinical Pak with prepaid postage affixed.

Step 5

Return the prepaid mailer to our lab by FedEx. You can do this by scheduling a free pickup either through fedex.com or by calling 1-800-GoFedex (1-800-463-3339). Unfortunately, FedEx office locations and FedEx drop boxes will not accept the mailer.
