Before you start

This kit is to be used for the following tests:

• Prequel^® Prenatal Screen

If the patient has had a transfusion within the past seven days, wait to collect the patient’s sample until seven days have passed.

Step 1

Write the patient’s full name, date of birth, collection date, and phlebotomist initials on the Streck tube. Also, label with gestational age.

Step 2

Draw blood from patient. Fill the Streck collection tube with at least a 7ml sample. After collection, gently invert sample tube several times. Vigorous shaking will compromise the specimen.

Step 3

Insert tube into the foam padding provided. Seal tube in the specimen bag. Place the specimen bag in the kit box. Give the patient card with the unique sample barcode to the patient for his or her records.

Step 4

Seal the kit box in the FedEx Clinical Pak with prepaid overnight priority postage affixed. Ship promptly.

Step 5

Return the prepaid mailer to our lab by FedEx. You can do this by scheduling a free pickup either through fedex.com or by calling 1-800-GoFedex (1-800-463-3339). Unfortunately, FedEx office locations and FedEx drop boxes will not accept the mailer.
