The Myriad Promise

Patients and their families use test results to make life-saving medical decisions. The Myriad Promise is our commitment to provide patients with access to accurate and affordable genetic results.

Insurance coverage

Myriad is in-network with most major insurance plans. For commercial and government-funded plans, Myriad will work with your health insurance company to obtain coverage for the test ordered by your healthcare provider.

Financial assistance

Many patients are eligible for Myriad’s Assistance Program

Financial Criteria

Myriad has a Financial Assistance Program that may be able to help if you are in a situation where the test may have a high cost due to coinsurance and/or deductible for your specific plan type.

Medical Criteria

Both insured and uninsured patients may qualify for free or reduced-cost testing if both financial and medical eligibility criteria are met. Download the information below to view financial criteria and medical criteria.

Payment Plans

If you do have any out-of-pocket costs for testing, Myriad offers interest-free payment plans for as low as $15 per month, until paid off. If you receive a bill, please contact us for help with enrolling.

Explanation of benefits (EOB) vs. patient bill:

In some cases, a patient might receive an Explanation of Benefits (EOB). An EOB is not a bill. It is a routine statement sent out by an insurance company with a breakdown of which treatments/services were covered. Patients with questions regarding their Myriad bill or insurance Explanation of Benefits should contact Myriad rather than their health care provider.
