Billing

  • What are my rights and protections against surprise medical bills?
Myriad is committed to accurate billing and does not intend to balance bill patients. The No Surprises Act protects you from balance billing for out-of-network services at an in-network hospital or ambulatory surgical center. Learn more here.
  • My insurance company informed me that I need prior authorization, what does this mean?
Prior authorization may be required by your insurance company to confirm medical necessity for screening. Our specialized team will submit prior authorization upon receiving your order as required by your insurance company. No action is required by the patient.
  • How much does screening cost?
Myriad Women’s Health has partnered with many health plans to provide patients affordable access to our genetic screenings. Each patient is provided an individualized price estimate via email and/ or text message within 48 hours of receipt of a completed order. Your out of pocket cost will be based on your provided medical information and insurance plan coverage. Myriad Women’s Health is dedicated to providing accessible testing. If cost is a concern, we offer financial options in our Myriad Access Program, including financial assistance and interest-free payment plans.
  • Can I use my Health Reimbursement Account (HRA), Health Savings Account (HSA), or Flex Spending Account (FSA) funds to pay for a screen from Myriad Women’s Health?
Services from Myriad Women's Health fall under the category of laboratory fees and are eligible medical expenses for HRAs, HSAs, and FSAs. You may use your HRA, HSA or FSA card as a form of payment with a valid credit line.
  • What is an ICD-10 (diagnosis) code?
The International Classification of Disease tenth revision (ICD-10) is a system of coding created by the World Health Organization that notes various medical records including diseases, symptoms, abnormal findings and external causes of injury. All U.S. healthcare providers are required to use this coding system. On October 1st, 2015, the healthcare industry  transitioned from ICD-9 to ICD-10. The new standard provides clearer explanation across the healthcare industry in regards to patient diagnosis. ICD-10's specificity increases the available codes to include over 68,000 codes to choose from.
  • Does Myriad Women’s Health offer financial assistance?
Myriad Women's Health works hard to make our screens as accessible as possible. If cost is a concern, once an order is submitted by your provider you can apply for financial assistance here. You will receive a follow up email confirming qualification if approved.  Please note each order requires a separate application to be submitted, therefore if your provider is ordering more than one screen, you'll need to submit multiple applications.   If cost is still a concern, we offer additional financial options in our Myriad Access Program, such as interest-free payment plans.
  • What is “maximum out of pocket?”
This is a yearly limit that a person has to pay for covered services according to her health plan. The following count toward this maximum out-of-pocket amount:
    • Deductibles
    • Copayments
    • Coinsurance
After you spend this amount on deductibles, copayments, and coinsurance, your health plan pays 100% of the cost of covered benefits. This limit does not apply to services that are not covered by the plan.  More questions about insurance terminology? Please review the resources below:
  • What is the “Estimated cost with insurance” referenced in the estimate?
The “estimated cost with insurance” is calculated  based on your health insurance plan, such as your remaining deductible, copay, and coinsurance. Click “How did you calculate my estimate?” towards the bottom of your price estimate for more detailed information.
  • Is Myriad Women’s Health in network with my insurance carrier?
Myriad Women’s Health is contracted as an in-network diagnostic laboratory with many national and regional insurers. We accept most commercial health insurance plans and strive to keep overall costs low. The term commercial insurance describes most employer-provided health plans but may not include government plans, such as Medicaid.   You can check with your health insurer if Myriad Women's Health is in-network by providing them with our tax identification number: 74-3238060.
  • I am not satisfied with my price estimate, can I cancel my screening?
Orders can be canceled, at no cost, up until the results are released to your ordering provider, by calling 888-268-6795. We encourage you to review our Myriad Access Program to explore your affordability options before cancelling.
  • What is the Myriad Promise?
We understand that every situation is unique, and we don't want cost to be a barrier. The Myriad Promise is our commitment to provide access to accurate and affordable screening to help patients make informed choices about their health, their families, and their futures.
  • Why is the financial responsibility on my explanation of benefits different from the price estimate I received from Myriad Women’s Health?
The Explanation of Benefits from your insurance company is not a bill or a final determination of your out-of-pocket costs. Your invoice is generated based on your insurance claim response, price estimate, and your qualification for the Myriad Access Program. Please refer to your Myriad issued invoice for your final cost.
  • I received an Explanation of Benefits for a claim from Myriad Women’s Health. When will I receive my invoice for any out-of-pocket costs?
Myriad Women’s Health generates invoices two to three weeks after receiving a final claim determination from your insurance company and may differ from the out of pocket cost on your Explanation of Benefits. Please note that your Explanation of Benefits is not a bill.
  • I cannot afford my price estimate, can Myriad Women’s Health help me with the cost?
At Myriad Women’s Health, we understand each situation is unique. Therefore, we have designed programs to make sure screening is affordable for everyone. The Myriad Promise is our commitment to provide patients with accurate and affordable genetic results. Please click here to learn about our Myriad Access Program that includes financial assistance and payment plan options.
  • What happens if my insurance denies my claim?
If you received communication from your insurance provider that your claim was denied, please wait for your invoice to understand your final out-of-pocket cost. If you have additional questions after you receive your invoice, please call 888-268-6795 and the choose the “Billing” option.
  • Why is my price estimate different than my partner’s estimate?
Estimated costs may vary between partners depending on multiple factors including specific plan benefits and the diagnosis codes indicated by the ordering provider. If cost is a concern, please call us at 888-268-6795 so our Prenatal Support Team can discuss affordability options offered through our Myriad Access Program.
  • What is considered the date of service for screening?
At Myriad Women’s Health, the date of service is the collection date (the date the sample was drawn).
  • My price estimate informed me I qualified for financial assistance, is further action required?
No further action is required if you qualified for financial assistance. Patients who meet criteria for reduced cost screening are enrolled into our financial assistance program. Patients can also submit an application manually and receive a determination instantly here. More information about our Myriad Access Program can be found here.
  • When should I expect to receive an invoice from Myriad Women’s Health for any possible out of pocket costs?
Self-pay patients will receive their invoice within 30 days after results are released. Patients who use insurance will receive their invoice after their claim is closed, typically 45-60 days after results are released.
  • Why do people with insurance have different out-of-pocket costs for the same screening?
Many individual factors are involved in healthcare costs and pricing. For those with insurance, your actual out-of-pocket cost will depend on:
    • The fraction of the total cost that your insurance covers, which varies by plan
    • The amount you have already paid for healthcare expenses this year
    • If your insurance provider is in-network or out-of-network with Myriad Women's Health (Tax ID: 74-3238060)
    • Meeting medical criteria to have insurance cover genetic screens
If cost is a concern, we offer financial options in our Myriad Access Program, such as financial assistance and interest-free payment plans.
  • What payment methods does Myriad Women’s Health accept?

  • Does Myriad Women’s Health offer payment plans?
Myriad Women's Health works hard to make our screens accessible. In an effort to make our screens accessible to patients regardless of their financial situation, Myriad Women's Health offers a variety of interest-free payment plans depending on the amount you owe.   Payment plans are available only for the full amount you are billed. You will need the following to set up a payment plan:
    • A completed screen from Myriad Women's Health
    • A Myriad Women's Health invoice for your screen
    • A valid credit/debit card
To set up a payment plan please call 888-268-6795 and choose the “Billing” option to speak with a representative.
  • Is it possible to get a screening without using my insurance?
If you are planning for other medical expenses this year, using your insurance may help you meet your deductible. That may mean less out-of-pocket expenses later on.  If you would rather not use your insurance to pay for your screening, please contact Myriad Women's Health to let us know not to file a claim with your health plan (or refrain from having your provider submit your insurance information when placing an order).  If you choose to pay without insurance the amount you will be billed by Myriad Women's Health will not be applied to your insurance benefits for the year. This may result in paying more for your healthcare expenses because you are not using your insurance benefits. The option to pay without insurance can only be chosen prior to filing a claim. Once a claim is filed, Myriad Women's Health is contractually obligated to complete that claim.
  • What is the Myriad Women’s Health Access Program?
We are committed to helping you make informed choices about your health, your family, and your future. We understand that every situation is unique. That’s why we’ve created Myriad Women's Health Access, a comprehensive program designed to make genetic screening accessible for more patients.   The Women's Health Access Program has three key components: Personalized cost estimates provided via email and/or text, in-network status with most health plans, and options for you to choose between to help you with your out-of-pocket costs, including financial assistance for those who qualify and interest-free plans.   To learn more about the Myriad Access Program, please click here
  • How do I pay my Myriad Women’s Health bill/invoice?
You may pay your Myriad Women's Health invoice online, by phone, or by mail.  To pay online, please click here. Once signed in, click the “My Bills” tab at the top of the page and enter your invoice number. Or, if you have your barcode number available, you may select the 'Register My Code' button at the bottom of the main page. Entering your barcode will register your screen to your account, which will allow you to view your balance due.  To pay by phone, please contact us at 888-268-6795, and follow the automated prompts to make your payment. Ensure that you have your invoice number readily available.  If you prefer to pay by check, please make the check payable to Myriad Women's Health and mail to:  Myriad Women's Health, Inc. P.O. Box 645719 Cincinnati, OH 45264-5719  We encourage you to contact us if you are unable to pay your bill in full. In keeping with our mission to provide access to valuable information, Myriad Women's Health will gladly work with you to keep our screens affordable.
  • What is an explanation of benefits (EOB)?
An explanation of benefits, or EOB, is a document that your insurance company sends to you to indicate how much money it has paid a healthcare provider (like Myriad Women’s Health) for services provided to you. The EOB also shows the amount due you may owe for those services (if your insurance did not cover it in full). An EOB is not an actual bill for you to pay. It is a notification to show how your insurance processed any claims for your Myriad Women’s Health services and typically requires no action from you.
  • How do I pre-verify my insurance coverage?
Myriad Women’s Health, we are dedicated to ensuring our screens are affordable for all patients. We provide individualized price estimates based on your specific order and insurance plan within 48 hours of receiving your sample at our laboratory. Price estimates are delivered by email and text message.  If you wish to obtain a precise estimate of your out-of-pocket costs prior to receiving a price estimate, please begin by calling the member services phone number on your health insurance card. Keep your card handy, as your insurer may request your Member ID. They may also request our tax identification number, which is 74-3238060, to determine our network status with your plan.   In order to receive a cost, you will need to ask about your plan's coverage for diagnostic lab testing. It may also be helpful to provide the billing (CPT) codes associated with the Myriad Women's Health screens, as well as our tax ID. CPT codes vary between insurance carriers and clinical indication codes. If you have questions about your specific CPT codes and have already sent in your order, click "Can I learn more about this test?" on your price estimate to view the codes that have been applied towards your screening.  If cost is a concern, we offer financial options in our Myriad Access Program, such as financial assistance and interest-free payment plans.
  • What is “other patient responsibility?”
In some cases, a health plan may not deem genetic screening as a covered benefit. In insurance terms, this is called “other patient responsibility.”   In these cases, the patient is responsible for the cost of the screen and the amount paid will not go toward the patient’s benefits, such as their deductible.   More questions about insurance terminology? Please review the resources below:
  • What does in-network mean?
If Myriad Women's Health is in-network with your insurance company, it means that we are part of your health plan's network of providers with which it has a negotiated price. This pricing will vary based on the screening options that were selected by your physician at the time of screening. The negotiated rate with your insurer is shown on your pricing estimate as the “Estimated cost with insurance.”  You can check with your health insurer if Myriad Women's Health is in-network with your plan by providing our tax identification number: 74-3238060.
  • How do ICD-10 codes affect my screen?
The diagnosis code is a method to communicate your provider’s medical reasoning for ordering your screening based on your current medical plan and history. The code is used to determine medical billing, but will not influence the processing of your sample.
  • What does out of network mean?
If Myriad Women's Health is out-of- network with your insurance company, it means that we are not part of your health plan’s network of providers and do not have a negotiated price.  Some health plans, like HMOs, may not reimburse out-of-network providers. This means that you, as the patient would be responsible for the full amount of the screen. In such cases, we offer the option to pay without insurance.   Other plans do offer coverage for out-of-network services under a separate out-of-network deductible and/or co-insurance rate. Out-of-network services typically do not apply to your standard in-network benefits.   You can check with your health insurer if Myriad Women's Health is in-network with your plan by providing our tax identification number, 74-3238060 and CPT codes associated with your order (these can be found on your individualized price estimate).  If cost is a concern, we offer financial options in our Myriad Access Program, such as financial assistance and interest-free payment plans.
  • What is Myriad Women’s Health’s Federal Tax Identification Number?
Myriad Women's Health's Federal Tax Identification Number (or "Tax ID") may be requested for billing purposes and is 74-3238060.
  • What is a CPT code?
A Current Procedural Terminology (CPT) is a billing code. It is an identifier given to a medical/diagnostic procedure that describes the services rendered. These codes are used by hospitals, physicians, and healthcare providers to communicate procedures or screens ordered to your insurance carrier.
  • What is “your responsibility?”
"Your responsibility" is on your price estimate to show the amount that has been designated towards your benefits by your insurer for the requested services. This may also be listed as "Estimated amount you will pay". This amount can include what you pay toward your deductible combined with any coinsurance or copay. Keep in mind that insurance will not pay until your deductible has been met. Any amount you pay for your in-network deductible, copay, and coinsurance will apply toward your annual out-of-pocket maximum. These amounts will vary depending on your specific plan and other services that have applied towards your benefits.  More questions about insurance terminology? Please review the resources below:
  • What does “covered” mean?
“Covered” does not necessarily mean that your health plan will pay the full cost of the screen. Instead, it means that the cost of the screen will apply to your benefits.   Your benefits include your responsibility for any of the following that you are required to pay under your specific health plan:
    • Deductible
    • Coinsurance
    • Copayment
    • Maximum out-of-pocket
  • What is coinsurance?
Coinsurance is a portion of the amount you are required to pay for a covered healthcare service. The percentage is based on your insurance plan as well as the service. Coinsurance only applies after you have met your deductible.   For example, let's say that your health insurance plan has the following:
    • Deductible of $500
    • Coinsurance of 10%
    • Copay of $20
Once you've met your deductible, you would be responsible for 10% of the cost of the covered healthcare service provided. Keep in mind that whatever you pay toward your coinsurance will be contributed toward your annual out-of-pocket maximum. Your actual costs and benefits may vary depending on your insurance company and your specific benefit plan.   More questions about insurance terminology? Please review the resources below:

  • Can you place a hold on my order?
No, we do not place samples on hold once they arrive for processing.
  • Why do I need physician approval to order a screen from Myriad Women’s Health?
While the regulatory landscape around genetic screening continues to evolve, genetic screens are clinical laboratory tests that require physician approval prior to screening under current law. • Can I send two kits back in the same FedEx remailer?
Yes, if you and your partner have both taken a screen from Myriad Women’s Health, please package each specimen in the individual Myriad Women’s Health collection box it came with before mailing both back in the same FedEx remailer.
  • Can I drop off my sample at Myriad Women’s Health?
No. This is due to health and safety regulations, Myriad Women’s Health cannot accommodate direct sample drop-off at our laboratory. Please follow the instructions included with your kit to return your sample via FedEx.
  • Why does my sample need to be recollected?
Sample recollections can be needed for a variety of reasons. Some possible reasons may include the following:
    • Blank tube (The specimen tube was unlabeled. Two patient identifiers are required to be written on the tube which may include patient name, date of birth, or barcode number)
    • Low specimen volume
    • Leaking sample
Insufficient DNA volume from your collected sample
  • What are the benefits of taking the Prequel® Prenatal Screen?
If you’re pregnant, the Prequel® Prenatal Screen is a noninvasive screen ordered by your healthcare provider that you can take as early as 10 weeks into your pregnancy to learn about your baby’s chance of having a chromosome abnormality leading to a condition like Down syndrome. All we need is a simple blood draw from your arm, causing no increased risk to you or your pregnancy. By analyzing small pieces of DNA from your baby’s placenta, called cell-free DNA, that float in the blood during pregnancy we can determine the chance that a baby has certain chromosome abnormalities. Results are available in approximately 1 week. The Prequel® Prenatal Screen has the lowest test failure rate in the industry, which translates to a lower chance of needing a repeat test or an unnecessary invasive diagnostic procedure. We use advanced science and technology that works for pregnant women of all ages,  and for those with twin and IVF pregnancies.

  • How does the ethnicity that my provider indicated impact my Prequel® Prenatal Screen?
For the Prequel® Prenatal Screen, ethnicity does not play any role in the risk to have an affected pregnancy  and it’s not part of the result’s positive predictive value or residual risk calculations.
  • Why is the gender of my baby not included on my Prequel® Prenatal Screen results?
The gender portion of the Prequel® Prenatal Screen, officially known as sex chromosome analysis, is optional and must be ordered by your provider. If you would like to add sex chromosome analysis to your order, please have your provider contact us and we will be able to add those results to your report within 48 hours. This request must come directly from your provider.
  • How are samples collected for the Prequel® Prenatal Screen?
A sample is collected from a pregnant patient who is at least 10 weeks (can be taken up to date of delivery) into her pregnancy through a routine blood draw. The blood draw can be completed at their provider’s office or any laboratory the office refers to for screening. Specifically, Myriad Women’s Health requires one 10 mL Streck tube (tan and black top), that should be gently inverted to mix the sample.
  • Who should consider the Prequel® Prenatal Screen?
The Prequel® Prenatal Screen is for any woman of any age who is at least 10 weeks pregnant and can be taken up to the date of delivery. The screen is validated for use in twin and singleton pregnancies, as well as pregnancies achieved by in vitro fertilization with or without the use of donor egg or sperm.
  • What is a microdeletion?
A microdeletion occurs when a tiny piece of a chromosome is missing or deleted. This deletion can result in birth defects or intellectual disability. Please click here to review which microdeletions the Prequel® Prenatal Screen can test for.
  • What is the turnaround time for the Prequel® Prenatal Screen?
On average, you can expect results approximately one week after the sample has been received by our laboratory. Please note, this quoted turnaround time is an average, so results may be ready sooner or later due to unique circumstances. For the fastest delivery, be sure to create a Myriad Women’s Health account to monitor the status of your results online.
  • My Prequel® Prenatal Screen results did not include sex chromosomes to determine gender, can I update my results to include this information?
If you recently received your results and sex chromosome analysis was not included on the report, and you would like this analysis, please advise your ordering provider to email [email protected] or call 888-268-6795. If the additional analysis is approved, an amended report will be generated within approximately 72 hours.
  • Can the Prequel® Prenatal Screen test solely for sex of the fetus?
No, the Prequel® Prenatal Screen cannot test solely for sex of the fetus. This test screens for common trisomies leading to conditions like Down syndrome. Your provider can choose to also include screening for the sex chromosomes, which can  predict whether a baby will be male (XY) or female (XX) and also can screen for sex chromosome differences. Sometimes, there may be too many or too few sex chromosomes, resulting in potential health issues. Sex chromosome screening cannot be ordered independently of common trisomies.
  • Can the Prequel® Prenatal Screen provide me with the gender results prior to receiving the rest of the results?
Because orders must fully complete the processing and review stages prior to release, we are unable to release partial results for our screenings, including the sex chromosome analysis, if ordered.
  • What is a Foresight® Carrier Screen couples report and how do I request one?
Myriad Women’s Health offers the ability to merge partners’ reports and provide a combined risk calculation for your Foresight® Carrier Screen. Your doctor can request to create a Foresight® Carrier Screen couple’s report by indicating it on your order form, or through our automated merge feature on their online Myriad Women’s Health account. We will be able to accommodate the request within 120 days of the original results completion date.   Please note we can only merge two individuals who have the same ordering physician.
  • I discovered I am a carrier after taking the Foresight® Carrier Screen. How can I order screening for my partner?
To order partner testing, please submit your request to your provider directly. Alternatively, you can follow the steps below:
    • Sign in to your account here, or sign up for a new account.
    • Once signed in, please visit the following link and select “Request a Screen” to begin the application.
After your application is submitted, a request for authorization of this screening will be sent to your ordering provider. Once authorization is received from your ordering provider, a kit will be shipped to your partner’s address.
  • How does the ethnicity that my provider indicated impact my Foresight® Carrier Screen?
For the Foresight® Carrier Screen. ethnicity is important because the chance you are a carrier for a specific condition and our disease-specific detection rates differ based on your ethnicity. There are certain conditions where ethnicity is important to determine whether you are a carrier or an “uncertain carrier.” Ethnicity does not change your genetic results, but it can change the interpretation of your results and your residual risk to be a carrier.  If you’d like to update the ethnicity on your report, please contact your ordering provider with your request. Change order requests must be submitted in writing by your ordering provider.
  • What is the next step if my Foresight® Carrier Screen results are positive?
The Myriad Foresight® Carrier Screen is intended to detect healthy carriers of rare inherited diseases. A positive finding means that you are a carrier, and the next step is to have your significant other undergo the same panel of screens. If you both screen positive for the same disease gene, your baby has a one in four, or 25 percent, chance of inheriting that condition. If you are a carrier, your siblings have a higher chance of being carriers of the same disease as well, so it is a good idea to let them know.   Our genetic counselors are available to discuss your results and clarify your choices. Please sign in to your account to book a phone consultation at no additional cost.
  • Are there any health conditions that can interfere with the accuracy of my Foresight® Carrier Screen results?
A history of bone marrow transplantation or recent blood transfusions can limit our ability to provide accurate results. We advise waiting at least two months after undergoing a blood transfusion procedure before proceeding with the Foresight® Carrier Screen.
  • What is the accuracy of the Foresight® Carrier Screen?
The Foresight® Carrier Screen is greater than 99.9% sensitive and specific for known gene changes that have been selected for inclusion in this screening. Patients have the option of utilizing blood or saliva collections kits when completing the Foresight® Carrier Screen.
  • What are the possible results of the Foresight® Carrier Screen?
The results generally indicate whether you are positive, or a carrier of a disease, or negative. Most of the conditions on the Myriad Foresight® Carrier Screen are inherited in a recessive manner. Through our merged results, you and your partner will discover the chance of transmitting certain health conditions to your child. Your chance will be represented in numerical format.
  • What is the turnaround time for the Foresight® Carrier Screen?
On average, you can expect results approximately two weeks after the sample has been received by our laboratory. Please note, this quoted turnaround time is an average, so results may be ready sooner or later due to unique circumstances. For the fastest delivery, be sure to create a Myriad Women’s Health account to monitor the status of your results online.
